Medications
All medications, prescription or over-the-counter, must be brought to school by a parent or guardian. Medication should NOT be sent with a student. A consent to give medication will need to be signed. Please feel free to print out the consent form linked below and bring it with you.
Medical Forms
All forms must be signed by a parent or guardian. Some forms require a physician's signature as well.

Is my child too sick for school?
A child should stay home if:
- there is a fever of 100°F or greater in the past 24 hours.
- vomiting and/or diarrhea occurred in the past 24 hours.
- he/she is too sick to be comfortable at school.
- he/she has a contagious illness/disease.
- he/she has a frequent, persistent cough.
- he/she has a persistent pain (stomach, ear, etc.) that will keep them from performing at school
A child should NOT return to school until fever, vomiting, and diarrhea has stopped for a full 24 hours without the help of medication. If a child has been diagnosed by a physician (strep throat, pink eye, etc.) and is being treated with antibiotics, they may return to school 24 hours after the antibiotics have been started.
